Colonial Acres Health Care Center at Covenant Village of Golden Valley

Providing either short-term rehabilitation or long-term care our team of highly experienced licensed therapists and nurses specialize in rehab for seniors. Available rehabilitation services include physical, occupational and speech therapy. Colonial Acres Health Care Center is a preferred provider for Park Nicollet Methodist Hospital and North Memorialís TotalCare for Seniors. Our rehabilitation services are offered both in-patient and out-patient at our campus.

Community Action Partnership of Suburban Hennepin (CAPSH)

Direct services, education and advocacy for low-income residents of Hennepin County. Services include: Energy Assistance, Food Support (SNAP), full-cycle Homeownership Programs, Homeless Transitional Services, Financial Literacy, Tax Assistance, Legal Services and MNsure Application assistance.
